Preparing for your visit:
- Please bring all pertinent medical records as well as imaging films and reports (x-rays, MRI’s, CT’s).
- Completed new patient information forms ( for new patients)

- Names and addresses of referring and primary doctors
- List and dose of all Medications that you are currently taking
What to expect when you see Dr. Shah:
- A thorough medical history will be taken
- A physical exam will be performed
- Urine toxicology will be performed based on the State compliance guidelines.
- All pertinent imaging studies and medical records will be reviewed and discussed.
- A comprehensive treatment or diagnostic plan will be offered.
